11/06/24 FIRST VISIT DR. JASON BOOLE (NW FLORIDA EAR, NOSE, & THROAT) DR. BOOLE PRESCRIBED LEVOFLOXACIN 100 MG FOR SINUS INFECTION. RECEIVED MEDICATION FROM MACK BAYOU COMPOUND PHARMACY & LEARNED ABOUT THE SERIOUS SIDE EFFECTS. DID NOT TAKE MEDICATION UNTIL I COULD TALK TO THE DR. ABOUT THE SERIOUS SIDE EFFECTS. 12/05/24 FOLLOW UP VISIT WITH JENNIFER BENDOKAS P.A. (NW FLORIDA EAR, NOSE, & THROAT) TOLD THE P.A. MY CONCERNS WITH THE LEVOFLOXACIN, SHOWING HER THE GOVERNMENT (BLACK BOX) WARNING FROM THE F.D.A. STATING THAT IT COULD CAUSE PERMANENT NERVE DAMAGE. SHE TOLD ME THAT IT WOULD BE OK FOR ME TO TAKE AND THAT I SHOULD START TAKING IT IMMEDIATELY. I WENT HOME THAT NIGHT AND STARTED TAKING THE MEDICATION AS PRESCRIBED. WITHIN 10 DAYS I STARTED EXPERIENCING EXCRUCIATING PAIN AND NUMBNESS IN MY ARMS AND HANDS. MY FINGERS STARTED TO CLOSE UP, MY UPPER BODY STARTED TO SEIZE, AND I LOST ALL FEELING IN MY FINGERS. 12/24/25 SAW DR BOOLE TO REPORT THE DAMAGE CAUSED BY THE LEVOFLOXACIN ONLY TO HAVE HIM SHRUG IT OFF AS A HYPERBOLE. I STILL HAVE EXCRUCIATING PAIN AND NUMBNESS IN MY ARMS AND HANDS THAT DOES NOT SUBSIDE. I SHOULD NOT HAVE BEEN PRESCRIBED THIS DRUG! PHYSICIANS ARE OBLIGATED TO FOLLOW THE HIPPOCRATIC OATH TO “FIRST DO NO HARM”. AGAIN, THERE WAS A BLACK BOX WARNING BY THE F.D.A. OUTLINING THE DANGERS OF THIS DRUG AND THAT IT COULD CAUSE PERMANENT NERVE DAMAGE, YET HE AND HIS PHYSICIAN’S ASSISTANT RECOMMENDED THAT I TAKE IT EVEN AFTER I BROUGHT THE STERN WARNING BY THE FOOD AND DRUG ADMINISTRATION TO THEIR ATTENTION. 2/1/25 PERMANENT NERVE DAMAGE I STILL HAVE EXCRUCIATING PAIN AND NUMBNESS IN MY ARMS AND HANDS THAT DOES NOT SUBSIDE. I CAN NO LONGER FEEL MY FINGER TIPS AND IT IS DIFFICULT TO WRITE, TYPE, WORK, OR SLEEP. 3/1/25 PERMANENT NERVE DAMAGE I STILL HAVE EXCRUCIATING PAIN AND NUMBNESS IN MY ARMS AND HANDS THAT DOES NOT SUBSIDE. I CAN NO LONGER FEEL MY FINGER TIPS AND IT IS DIFFICULT TO WRITE, TYPE, WORK, OR SLEEP.
