Absolutely the worst experience. I have been a patient of Dr Hazen’s for years. Had few years break and had recently tried returned. The staff in the front refused to confirm my insurance, said today would be self pay. Although I had all insurance information. Extremely sad experience. I just sat in my car and cried.
